Who we are

Primary and Community Health (PaCH) provides health services in people's homes, in early childhood centres, community health centres, and other community locations. PaCH provides acute, subacute and community health services in partnership with general practitioners (GPs), private allied health services, residential aged care facilities, independent Aboriginal health services and other providers.

 

Services provided by PaCH are organised under six broad clinical streams including:

  • Aged and Chronic Care
  • BreastScreen NSW
  • Child Youth and Family Health,
  • Northern Sydney Home Nursing Services,
  • Oral Health,
  • Population Health that encompasses Intellectual disability, sexual health, needle and syringe, and multicultural health services

 

PaCH services have four core functions:

  • Assessment, referral and episodic/short term treatment for common health conditions
  • Prevention, early detection and intervention for health problems or health risks
  • Ongoing care of chronic and complex conditions in collaboration with specialist services
  • Acute care in the community.
