Consumers (patients, clients, carers) who are more active participants in their own care are better able to navigate the health system. The following links and resources may help you improve decision making and ask the right questions about treatment and care to improve your healthcare experience and outcomes. You have an important role to play by partnering in your care collaboratively with healthcare professionals to make informed decision on your health and wellbeing.

    REACH is a system that helps patients, their family and carer/s escalate their concerns with staff about worrying changes in a patient's condition. It stands for Recognise, Engage, Act, Call, Help is on its way.

     

    If you are worried about a patient in hospital, first talk to your nurse or doctor. If you are still worried, ask to speak to the nurse in charge and request a 'clinical review'. If you are still worried, call your hospital's REACH number listed below.
