UPDATE: TERM 1, 2021 - COURSES SUSPENDED
Unfortunately, Healthy Lifestyle classes will remain suspended for Term 1, 2021.
We appreciate the value of these classes to our participants and sincerely apologise for this continued disruption.
Northern Sydney Local Health District is committed to ensuring the safety of our community. In light of the recent increase in COVID-19 cases in the Northern Sydney region, the risk of convening group exercise classes for potentially vulnerable participants remains high.
We are aiming to resume the Healthy Lifestyle program (using venues that comply with COVID-19 safety requirements) in Term 2, commencing 19th April.
All Term 1 enrolments will be rolled over to Term 2. If you wish to withdraw your enrolment please email the Health Lifestyle team

How to Enrol
Please read the
booking conditions carefully before proceeding:
ONLY ONE PERSON/CLASS
SELECT the course you would like to attend.
HAVE your doctor complete the Healthy Lifestyle Medical Clearance Form (if required).
COMPLETE the Enrolment Form and emergency contact details.
ATTACH completed and signed Medical Clearance Form (if required).
COMPLETE the credit card payment details on the Enrolment form or make cheques/money orders payable to: Northern Sydney Local Health District.
SUBMIT your completed Enrolment Form, together with the Medical Clearance Form (if required) and the fee (cheque or credit card) by

(Note: the online PDF Enrolment Form, in the Enrolment Section, can be completed and emailed from your computer. Please download the form to your desktop computer, then open the form using Adobe Reader. Completing the form via your web browser may not allow the controls/buttons to work correctly).

All Healthy Lifestyle participants will be COVID-19 screened.
Healthy Lifestyle will not be offering make up classes or credits.
No re-enrolments (rolling enrolments from term to term) will be offered.
- Healthy Lifestyle reserves the right to determine whether a participants is appropriate for the course in which he/she has enrolled. Where possible, a more suitable course will be offered.
- You may enrol by email, fax or mail.
Enrolments are processed on a first come first serve basis.
- Receipts are emailed (only posted if necessary) after your enrolment has been processed. Please allow adequate time before enquiring.
- Some courses require a Medical Clearance. Use the medical clearance provided by Healthy Lifestyle and have it completed by your doctor.
- If your class required a medical clearance, please ensure your signed form is attached to your enrolment, otherwise your enrolment will not be processed.
- Classes are NOT held on public holidays.
- Further terms and conditions are covered on receipts and enrolment forms.
Participants are encouraged to explore the potential for rebates with their private health insurer.
The Healthy Lifestyle program is a non-registered provider for the National Disability Insurance Scheme (NDIS), NDIS participants may be able to use self-managed funds to claim the cost of attending a Healthy Lifestyle course if it is included in their plan prior to registering.
Refunds, Credits and Cancellations:
Healthy Lifestyle does not accept responsibility for participants' non-attendance of classes due to changes in personal or business circumstances/commitments /holidays during the term. Healthy Lifestyle is unable to offer make up classes or credits.
If Healthy Lifestyle cancels a course and a suitable alternative is not available, a full refund of your course fee will be issued. Also if Healthy Lifestyle cancels an individual class/es due to leader illness or a venue issue, we will endeavour to find a replacement leader/venue. If it is not possible to secure a replacement Leader or venue a refund will be issued. Please allow 4 to 6 weeks for processing.
Participants who withdraw from a course before it begins, will not receive a refund unless notice is received at least 5 working days prior to the start of the course. An administration fee of $20 per participant will be charged.
In the event you are unwell and unable to attend your class/es a refund may be requested. Refund requests will be processed for
medical reasons ONLY with an accompanying medical certificate. An administration fee of $20 will be charged.
