The Virtual Palliative Care Navigator Service is a helpline designed to support health professionals in Northern Sydney to find and access palliative care services for patients with life-limiting illnesses. Led by Clinical Nurse Consultants, the service facilitates timely access to specialist care, advice, interventions, and referrals. It supports a range of healthcare providers, including paramedics, general practitioners, and community health professionals.
The Virtual Palliative Care Navigator Service:
- Offers a contact point for healthcare providers to find and access supportive and palliative care services across NSLHD
- Coordinates communication between healthcare providers and emergency services to ensure smooth transitions of care
- Assists healthcare providers in identifying pathways and referring people with palliative or end-of-life care needs
