Virtual Palliative Care Navigator Service

The Virtual Palliative Care Navigator Service provides access to a Palliative Care Clinical Nurse Consultant between 9 am and 5 pm Monday to Friday via a helpline. The service offers care navigation and consultation regarding supportive and palliative care services across NSLHD to:​

  • Navigate care pathways between community services and acute settings
  • Offer a single point of contact and support to general practitioners (GPs) and health practitioners regarding acute issues in supportive and palliative care for patients in the community
  • Enhance access to supportive and palliative care advice and referrals to reduce unnecessary emergency department presentations and acute hospital admissions
  • Support community-based palliative care management to ensure the right care in the right place​

 Eligibility and referrals

Eligibility:

Anyone 16 years or older living in the NSLHD catchment with an active, progressive life-limiting illness requiring palliative care assessment or support.

Referrals:

The Virtual Palliative Care Navigator Service helpline is open to NSW Ambulance, GPs, medical specialists, and other community healthcare providers.
