The Chronic Disease Community Rehabilitation Service (CDCRS) is a multi-disciplinary service providing an evidenced-based approach to the management of individuals with chronic respiratory disease and/or heart failure. Members of the multi-disciplinary team include: a Respiratory Staff Specialist, Physiotherapists, Clinical Nurse Specialists, Clinical Psychologists, an Occupational Therapist and a Dietitian.

Interventions provided by the team comprise exercise rehabilitation, sputum clearance, psychological management, Pulmonary Rehabilitation Physician review, medication review, nutritional management, home modifications/aids, disease-specific education.

Eligibility

Patients with a confirmed diagnosis of chronic respiratory disease and/or heart failure who would benefit from the abovementioned interventions.

Additional eligibility criteria for exercise rehabilitation

  • No significant musculoskeletal, neurological or cognitive impairment that would preclude the ability to participate in group-based exercise.
  • Absence of symptomatic cardiac disease, where exercise would be unsafe.

Referral can be made through the Northern Sydney Local Health District Health Contact Centre on 1300 732 503 (Monday to Friday, 7:30am-6:30pm).
