Telephone support is available from qualified health professionals with chronic disease management expertise. During telephone calls patients receive encouragement and assistance to navigate services that will address their complex health and social needs. The service aims to enhance a patient's self-management capacity and achieve optimal physiological health outcomes.



To be eligible for the telephone support service, patients must:

1. Consent to be referred to the service.

2. Be aged 16 years or older.

3. Have a diagnosis of Hypertension, Coronary Artery Disease, Diabetes, Heart Failure or Respiratory Disease.

4. Live, work or get admitted to hospitals within Northern Sydney Local Health District.

5. Have access to a telephone and the ability to communicate effectively by telephone (interpreters will be arranged).


Self-referrals are accepted, as are referrals from health professionals such as your GP, Specialist or Community Home Nursing. To refer, phone NSLHD Health Contact Centre directly, on 1300 732 503.
