A ComPacks package is a non-clinical case managed package of community care available for people being transferred home from a participating NSW public hospital.

Each package is for a maximum of six weeks and is for those patients who require immediate access to case management and at least two community services to safety return home from hospital.

Typical services include case management and a mix of personal care, domestic assistance, transport to medical appointments and social support

Care Connect is the contracted NSLHD ComPacks provider.

There are an allocated number of ComPacks packages available each week, with more in the winter months and less during the summer months.

ComPacks is based on a collaborative model of care between NSW Ministry of Health, Local health Districts and brokered community organisations.

Eligibility

The patient has been assessed as requiring the following to enable transfer home from hospital:

  • Case management and a combination of low level community support services such as personal care, domestic assistance

  • The patient does not have access to or have in place existing community support services such as DVA, existing HACC services, a Home Care package

  • No other informal support options are available to the patient eg family or other carers

  • Note: an ACAT assessment is not required.

Referral

Referrals are made by staff from a participating public hospital to Care Connect Intake Service and either faxed to 02 9310 5722 or scanned and emailed to compacks.intake@careconnect.org.au and must include:

  • NSLHD ComPacks Fax Cover Sheet
  • ONI Form.

Referrals can be made by staff in the following NSLHD participating hospitals/services:

  • APAC
  • Hornsby Ku-ring-gai Hospital
  • Healthy at Home
  • Mona Vale Hospital
  • Royal North Shore Hospital
  • Ryde Hospital
  • Greenwich and Neringah Hospitals
  • Royal Rehabilitation Centre Sydney (RRCS).

Urgent cases

ComPacks does NOT provide urgent care but a referrer may ring the Care Connect intake number 1800 909 551 to discuss an urgent referral.
