Multidisciplinary, goal based rehabilitation program completed in the home or community environment. Program is overseen by a Medical Rehabilitation Physician. 

Disciplines available include physiotherapy and occupational therapy. Goals may include community access retraining; public transport retraining; falls prevention in the home environment; outdoor mobility; progressing and upgrading balance and walking aids; carer training.  

Prescription of a home exercise program is included to patient and carer for ongoing maintenance of function.

Long term maintenance program such as hydrotherapy and fitness gym, if appropriate, can be provided within the service at Greenwich Hospital.



  • Live within NSLHD area
  • Aged over 16yrs
  • Medically stable and suitable home environment
  • Identified goals that can be reasonably achieved in a six week period
  • Requirement of either physiotherapy, occupational therapy or both
  • Medical Specialist, GP or other appropriate health professional referral
  • Consent to treatment
