Acute Care team provides home-visit based clinical services to people living in the community. The clinical service includes mental health assessments and short-term interventions in response to mental health crisis or emergency situations; as well as care coordination for people with longer-term chronic mental illness. The team works with adults aged 18 – 65 years.

Eligibility

Age 18 – 65 years with a mental illness or disorder, who would benefit from mental health acute crisis support or case management.

Referral

People can self-refer, or health professionals can refer on their behalf, by phoning the Mental Health Line on 1800 011 511.

Urgent cases

There are several options to access urgent mental health care:

Ring the intake line on 9477 9520. This line is staffed 24 hours a day, 7 days a week. You will be able to speak with a clinician who can provide recommendations about how to manage the situation or arrange for an assessment.

In urgent cases you can attend the Emergency Department at Hornsby Hospital where there is 24 hour access to specialist mental health care and support.

If the situation is urgent or becoming dangerous ring Triple Zero (000) and request ambulance and/or police. They will be able to assist the person safely to the hospital Emergency Department to access mental health care.

The clinical team is present 7 days, 8.30am – 10pm daily; the phone services are staffed 7 days, 24 hours per day.
