SRCs support the Case Management role and provide direct services to consumers to promote personal recovery and enable people with mental illnesses to live their lives to the fullest potential. SRCs work within the evidence based framework of the Collaborative Recovery Model. The team liaise closely with Care Coordinators and any other clinicians involved around their contact with consumers to ensure coordinated care and avoid service duplication. SRCs are also available as a resource to all adult mental health teams and are available for consultation regarding individual consumer recovery, including access to referral pathways to external recovery support services.

Eligibility

  • Aged 18 – 65 years.
  • Live in Northern Beaches catchment area.
  • Have a diagnosed mental illness.
  • Have a current Care Coordinator in the Northern Beaches adult mental health service.

Referral

Referrals are accepted from care coordinators from adult mental health teams, for consumers living in the community.

Urgent cases

SRCs do not provide a crisis / acute service.

If you require urgent assistance there are several options:

Ring the Mental Health Line on 1800 011 511. This line is staffed by experienced clinicians 24 hours a day, 7 days a week. You will be able to speak with a clinician who can provide recommendations about how to manage the situation or put you in contact with the appropriate mental health team.

If the situation is urgent or becoming dangerous ring Triple Zero (000) and request ambulance and/or police. They will be able to assist the person safely to the hospital Emergency Department to access mental health care.

What is meant by personal recovery?

Recovery is a broadly defined term. If viewed in a clinical sense it relates to cure and or symptom reduction/ relapse prevention. Personal recovery may or may not include clinical recovery. It is about supporting consumers towards creating a life with meaning, purpose and hope, even with challenges associated with mental illness. Personal recovery is not dependent on consumers having insight into their condition.

What is the Collaborative Recovery Model?

It is a model designed to incorporate evidence based practices to assist people living with enduring mental illness to increase and promote their sense of wellbeing and quality of life. The process is collaborative and individualised in nature. It begins with ascertaining the consumer's current stage of psychological recovery (Moratorium/hold, Awareness, Preparation, Rebuilding, and Growth) and incorporates various components such as motivational interviewing, identification of personal strengths and values, goal setting, action planning and review to support personal recovery. It also includes a range of Assessment and Outcome measures.
