Hospital in the Home (HITH) - Formerly Acute Post-Acute Care (APAC)

The Hospital-in-the-home service provides short-term acute health care instead of care which would otherwise be provided in hospital.

The program cares for patients who:

• are medically stable
• have access to a telephone
• are being treated in the Northern Sydney area
• have a doctor who agrees to be medically responsible.

Referrals to HITH are are made via the number 1300 790 790

General Practitioners (GPs), specialists, hospital emergency departments and public and private hospitals can refer patients to HITH.

Patients and residential aged care facilities, with agreement from a GP, can also request services from HITH
