- At risk of presentation or re-presentation to a hospital due to acute functional or cognitive decline
- High falls risk or recurrent falls
A referral from a GP is required to see the doctor and team physiotherapist.

All people referred are treated as urgent cases and will be contacted quickly after receiving the referral. The team aims to see each patient within 7 days.
An important part of the service is outreach to Residential Aged Care Facilities (RACFs) to support RACF staff in managing the care of older people requiring specialist or hospital services. The service is similar to the GRACE service at Hornsby Ku-Ring-Gai Hospital.
There are falls prevention clinics for community dwelling older people at Mona Vale Community Health Centre on Wednesday mornings and at Manly Hospital on Thursday mornings. Referrals can me made via the same fax number.
