Services offered include assertive follow up and case management to people aged 18 to 65 years who are experiencing a severe and enduring mental illness and are living in group homes, Department of Housing properties and private residences.

The team is staffed by multidisciplinary health professionals including social workers, registered nurses, psychologists, occupational therapists, psychiatric registrars and psychiatrists.

The focus of outreach support is to assist people to develop skills necessary to maintain living in their accommodation, improve social connectedness and quality of life.



  • Aged: 18 - 65 years of age
  • Area Covered : All areas of Northern Beaches
  • Experiencing mental illness or disorder


Referrals are only accepted from Community Mental Health staff, Northern Beaches Hospital and Macquarie Hospital.

Urgent cases

  • Ring the Mental Health Line on 1800 011 511. This line is staffed 24 hours a day, 7 days a week.
  • In urgent cases you can attend the Emergency Department at Northern Beaches Hospital where there is 24 hour access to specialist mental health care and support.
  • If the situation is urgent or becoming dangerous ring Triple Zero (000) and request ambulance and/or police.
