This team provides an intensive service to people with severe mental illness to help support the recovery process. It provides care coordination, education, support, advocacy and rehabilitation for its clients. It aims to instil hope, promote wellbeing and support recovery. The team works with adults aged 18 – 65 years.

Eligibility

  • Live in the lower North Shore catchment area.
  • 18 – 65 years of age.
  • The person has severe and enduring mental health needs (e.g. a diagnosis of schizophrenia / major affective disorder).
  • The person has a history / is at risk of a high use of in-patient care (e.g. more than two admissions or more than six months inpatient care in the past two years).
  • The person has difficulty in maintaining lasting and consenting contact with services.
  • The person has multiple and complex needs including (a number) of the following:
    1. History of violence or persistent offending;
    2. Significant risk of persistent self-harm or neglect;
    3. Poor response to previous treatment;
    4. Dual diagnosis of substance misuse and serious mental illness;
    5. Unstable accommodation or homelessness.

Referral

Health professionals need to complete the AOT Referral Form and provide to the team along with the supporting essential documentation and other relevant reports and assessment result.

Urgent cases

There are several options to access urgent mental health care:
Ring the Mental Health Line on 1800 011 511. This line is staffed 24 hours a day, 7 days a week. You will be able to speak with a clinician who can provide recommendations about how to manage the situation or put you in contact with the appropriate mental health team.

In urgent cases you can attend the Emergency Department at Royal North Shore Hospital where there is 24 hour access to specialist mental health care and support.

If the situation is urgent or becoming dangerous ring Triple Zero (000) and request ambulance and/or police. They will be able to assist the person safely to the hospital Emergency Department to access mental health care.
