• Catheter issues
• Peg tube issues
• Hospital avoidance / deteriorating residents
• Skin diseases
• Education and clinical support for facility nurses
• Case co-ordination, in particular for complex hospital discharges into aged care facilities
• Case discussion with GPs

We care for frail elderly people living in aged care facilities on the Northern Beaches.

• List of current medications
• Referral

Must be a resident of an aged care faciity on the Northern Beaches at risk of hospital admission due to deterioration.

• GP referral
• Nursing referral 
• Medical referral
