What we do

The cardiac rehabilitation service offers the following for anyone who has recently had heart problems, such as a heart attack or cardio-vascular surgery:
• Individual assessment
• Education sessions
• Exercise programs

 

The cardiac rehabilitation program will help participants to:
• Understand heart disease
• Learn about treatment options
• Adopt a more healthy lifestyle
• Remain active

 

Heart failure service

The heart failure service helps people with heart failure and assists them to manage their condition.

 

Patients can attend outpatient clinics or receive home visits and/or regular phone calls:
• Clinical assessment
• Education on how to manage heart failure
• Education on diet and lifestyle
• Exercise program
• Care plan with GP

What to bring

• Cardiologist and GP contact details
• List of all your medications

When to visit us

• After a recent admission to hospital for a cardiac event
• Referral from GP or cardiologist
• Over 18 years of age
• Resident of Ryde or Hunters Hill local government areas

How to visit us

.

Ryde Hospital provides 350 free car parking spaces spread across six car parks. The main car park comprises both covered and open air parking spaces via Denistone Road. The other large car park is a covered car park in the basement of the Graythwaite Rehabilitation Centre via Fourth Avenue. There is a patient drop off zone outside the emergency department.

Referral information

Referral required from GP or Cardiologist.

Exclusions: this program is not appropriate for people with severe dementia.
