The heart failure service helps people with heart disease to control their condition and remain active.
Patients can attend outpatient clinics or receive home visits and/or regular phone calls:
• Clinical assessment
• Education on how to manage heart failure
• Education on diet and lifestyle
• Exercise program
• Care plan with GP
