ComPacks – home support services

ComPacks is a package of services provided to help people who need support to return home safely after a hospital admission. A combination of community case management and non-clinical community services are provided and may include assistance with personal care, domestic assistance, transport and social support, and is available for up to six weeks from the time of discharge from hospital.

ComPacks is available to all patients who meet the eligibility criteria, irrespective of age. An Aged Care Assessment Team (ACAT) assessment is not required. ComPacks is funded to support public patients discharged from participating hospitals in the NSW Health System. Referrals cannot be accepted for patients from Private hospitals.

ComPacks services are provided to people living in Northern Sydney Local Health District (NSLHD) by CareConnect.

Patients eligible to be referred for a ComPacks package when leaving hospital include:

A patient who requires immediate access to case management and community support services.

A patient who does not have services in place or cannot be accessed immediately through other channels like My Aged Care, Home Care Packages, Commonwealth Home Support Program, Department of Veterans Affairs etc.

Where no informal support options are available for the patient.

Inpatient social workers will need to seek approval to access the ComPacks portal in order to make referrals. This can be done by sending an email to: compacks.intake@careconnect.org.au, with the following information: Name, Facility, Role, Email, Phone number, Extension / pager.

Once permission has been granted and login details have been provided the Compacks referral portal can be found on the CareConnect website https://www.careconnect.org.au/

Cost: Clients may be asked to pay a nominal fee (capped at $10 per week).
