Patients eligible to be referred for a ComPacks package when leaving hospital include:
A patient who requires immediate access to case management and community support services.
A patient who does not have services in place or cannot be accessed immediately through other channels like My Aged Care, Home Care Packages, Commonwealth Home Support Program, Department of Veterans Affairs etc.
Where no informal support options are available for the patient.
Inpatient social workers will need to seek approval to access the ComPacks portal in order to make referrals. This can be done by sending an email to: email@example.com, with the following information: Name, Facility, Role, Email, Phone number, Extension / pager.
Once permission has been granted and login details have been provided the Compacks referral portal can be found on the CareConnect website https://www.careconnect.org.au/
