What we do

We care for and treat heart failure patients recently discharged from hospital to home.

 

The heart failure service provides home visits for patients following an episode of acute decompensation of heart failure. We provide continued support to patients and their carers with regular telephone calls for up to 12 months. The nurse provides comprehensive patient assessments and education regarding modifiable lifestyle behaviours, symptom recognition, and self-management.

Hornsby Hospital has a multi-storey car park that offers 500 spaces and is located at gate 2 on Palmerston Road.

Parking fees apply across the hospital in accordance with the NSW Health's Hospital Car Parking Fees Policy. Concessional rates apply to those who are eligible. Disabled parking spaces are available.
