What we do

The heart failure service provides a home visit following an episode of acute decompensation of heart failure and follow on support to patients and their carers with regular telephone contact for up to 12 months in the community. The nurse provides a comprehensive patient assessment, education for heart failure patients regarding modifiable lifestyle behaviours, symptom recognition, and supports self-management.

 

We care for and treat heart failure patients recently discharged from hospital to home (excluding nursing home residents).

When to visit us

How to visit us

For access to the Mona Vale community health building please access the hospital car park via Gate 3.

Free parking is available at Mona Vale Hospital for all visitors and staff within the designated car parks on the hospital grounds. There is two hour street parking also available surrounding the hospital grounds.

For access to the Beachside Rehabilitation Unit, hydrotherapy pool, loading dock and support services please enter the hospital car park via Gate 1.

For access to the Urgent Care Centre please access the hospital car park via Gate 2.

Referrals can be from nursing, medical, allied health and other health services.
