What we do

The heart failure service provides a home visit following an episode of acute decompensation of heart failure and follow on support to patients and their carers with regular telephone contact for up to 12 months in the community. The nurse provides a comprehensive patient assessment, education for heart failure patients regarding modifiable lifestyle behaviours, symptom recognition, and supports self-management.

 

We care for and treat heart failure patients recently discharged from hospital to home (excluding nursing home residents).

How to visit us

Ryde Hospital provides 350 car parking spaces spread across six car parks.

The main car park comprises both covered and open air parking spaces via Denistone Road. The other large car park is in the basement of the Graythwaite Rehabilitation Centre via Fourth Avenue. There is a patient drop off zone outside the emergency department.
