 What we do

The heart failure service provides a home visit following an episode of acute decompensation of heart failure and follow on support to patients and their carers with regular telephone contact for up to 12 months in the community. The nurse provides a comprehensive patient assessment, education for heart failure patients regarding modifiable lifestyle behaviours, symptom recognition, and supports self-management.

 

We care for and treat heart failure patients recently discharged from hospital to home (excluding nursing home residents).

Turn off Blaxland Road onto Florence Avenue then turn right onto Denistone Road. Both the main entrance and emergency department are located here.

Catch bus 515 from Eastwood Station to the city, which stops outside the hospital.

Get off the train at Eastwood Station. From here it is a one kilometre walk to the hospital. Alternatively you can catch bus 515 from Eastwood Station to the city, which stops outside the hospital.

The hospital is about a one kilometre walk from Eastwood Station.

There are a limited amount of free disability and outpatient visitor parking spots available on-site during hospital redevelopment construction. Please consider being dropped off. There are drop off zones at the main entrance off Denistone Road and in front of Graythwaite Rehabilitation Centre off Fourth Avenue. Alternatively allow for additional time to find on street parking.

The Clinic is located in the Outpatient Area on level 1. When you enter the hospital at the main entrance turn immediately right down the corridor. Stair access to outpatient area is on the left at the end of the corridor before you get to the Emergency Department waiting room. 


Lift access to the outpatient area is on your left-hand side as you enter the Emergency Department waiting room. Take the lift to level 1 and you will face the outpatient clinic area.

  • Referrals can be from nursing, medical, allied health and other health services

Exclusion Criteria:

  • This program is not appropriate for nursing home or palliative care patients.

 

Please submit referrals using the eReferral system through HealthLink, which can be accessed through most clinical software systems.

 

Referrals will be triaged and your patient will be notified of the outcome as soon as possible. This process may take between 1-2 weeks. You will be contacted if more information is needed.
