What we do

Welcome to our multidisciplinary diabetes service. A specialised service for adults living with diabetes mellitus.

We are here to assist people with education and understand the management of diabetes.

We provide specialist consults with friendly, professional and supportive diabetes doctors (endocrinologists), ADEA credentialled diabetes nurse educators and a part time diabetes specialised dietitian.

We offer a Young Adult Clinic YAC (16 years – 25 years)

We promote and assist people with self-management and improved understanding of:

  • Type 1, Type 2 & Type 3c diabetes
  • Complications from diabetes
  • Complex diabetes treatments
  • Insulin stabilisation and training in the use of insulin/GLP 1 delivery systems
  • People requiring steroid treatment which can impact on blood glucose levels.
  • Training in the use of sensor and blood glucose monitoring systems
  • Dietary information and carbohydrate counting needs. (Currently not offered)
  • Young Adults Clinic (16 – 25-year-old)
  • Monthly Diabetes Group Education Program for people with a new diagnosis of Pre-Diabetes & Type 2 Diabetes.
  • Diabetes Support Group for those with Pre-Diabetes / Type 2 Diabetes

What to bring

  • GP or specialist doctor referral 
  • Blood results
  • List of current medications
  • Blood glucose record and testing equipment.
  • Insulin pump

When to visit us

All patients referred to the service must reside within the Northern Beaches Local Government Area

How to visit us

Free car parking is available at Brookvale Community Health Centre for all visitors. Park in on the ground floor commuter car park off Pittwater Road (Turn at PetO sign) or William Street. Do not proceed up the ramp through boom gate or a charged will be incurred. 
Alternatively park in Warringah Mall and walk across to Brookvale Community Health Centre.

Please note: all referrals will be triaged by our Diabetes Service Team and based on the information contained in the referral patients will be offered the most appropriate appointment

 

Referral to Endocrinologist - Diabetes Doctor Clinic Medical referral are required to contain patient name, date of birth, contact details. Medical history, recent blood results, reason for referral. Please refer to:

Referral to Diabetes Educator:

Medical referral, Nursing referral, Allied Health Referral 
All referrals are required to contain patient name, date of birth, contact details. Medical history, recent blood results, reason for referral.

The services of an Interpreter for our Culturally and Linguistically Diverse clients can be organised if required.

Patients with cognitive or behavioural issues must be accompanied by their carer.

Group Education Programs:

 

Diabetes Education Program


Group Education is run once per month at each site (Brookvale and Mona Vale) for newly and previously diagnosed clients with type 2 diabetes or prediabetes. Topics covered in this program include up to date, relevant and practical information on diabetes and prediabetes management to assist patients to manage their condition. The program is run in conjunction with the Dietitian.

 

Diabetes Peer Support Group: Offered every three months for people who have attended Diabetes Education Program.
