By Appointment Only and excluding Weekends and Public Holidays. 
 

 What we do

We provide treatment for persons aged 18 years and above with short-term rehabilitation goals, living at home in the Northern Beaches Local Government Area, excluding National Disability Insurance Scheme/Worker’s Compensation/Compulsory Third Party participants, Transitional Aged Care Package recipients and nursing home residents. 

 

Clients’ service needs may be related to a recent hospital admission, neurological condition, upper limb impairment due arthritis or stroke or those recovering from an injury or surgery. Common client goals include improving functional ability in activities of daily living. 

 

Home-Based Rehabilitation services can be offered to clients over 65 years through the Commonwealth Home Support Program (CHSP) funding for Wellness and Reablement. Clients may include people with disabilities and frail aged needs that can be addressed in up to 6 sessions delivered in our clinics or in the client’s home.

 When to visit us

Your GP or non-GP specialist is always your first step in seeking treatment for any condition. You will be referred to our service if your treating doctor feels that your treatment requires the input of an Occupational Therapist (OT). 

 

Once we receive the referral it will be reviewed by the OT and if accepted, you will be notified of the next available appointment. We see clients face to face but may recommend telehealth in certain situations, which your OT will arrange with you. 

 

Please note that phone calls from the hospital will appear as a blocked number.  Please answer our call if you can and/or turn on voicemail to allow us to leave you a message.  If you have a scheduled appointment, you will receive a reminder text message to your mobile phone.   


 
Allow approximately 2 hours for your first appointment. On arrival, please wait in the designated seating area on Level 3 at the Community Health Centres and your OT will call your name at your appointment time. 


Free parking is available on site and in neighbouring streets, however it is limited, and we recommended you allow for travel time.  Public transport options (buses) are also available to attend both clinics.  
 

 How to visit us

Mona Vale: When travelling north along Pittwater Rd, turn right at Coronation St. When travelling south along Pittwater Road, turn left at Coronation Street. Parking is available on site.

 

BrookvaleWhen travelling south along Pittwater Rd, the Community Health Centre is located on the left hand side opposite Warringah mall, just before the turn off for William Street. Parking is available on site.

Mona Vale: Bus route 155 alights in front of the hospital, while routes 151, 160X, 188, 188X, 189X, 190X and 199 stop at the intersection of Pittwater Road and Coronation Street.

 

BrookvaleNumerous bus routes, including B1 and 199, stop at the bus terminal directly outside the Centre. 

Mona Vale: The walk from the bus stop at the intersection of Pittwater Road and Coronation street is approximately two minutes or 250m.

 

Brookvale: If coming from the Warringah Mall side of Pittwater Road, use the Pedestrian bridge to cross the street. The bridge has lifts to accommodate  passengers, bicycles, wheelchairs and prams.

Mona Vale: Free parking is available at Mona Vale Hospital for all visitors and staff within the designated car parks on the hospital grounds. There is two-hour street parking also available surrounding the hospital grounds.

 

Brookvale:  There is designated parking at the Community Health Centre (free on the ground floor, paid parking on levels 1-5 of multi-storey car park).

Mona Vale: When arriving to the community health centre, please use the lifts inside the main entrance to proceed to level, 3, where our clinic is located. Please take a seat in the level 3 waiting area.

 Referral information

To ensure that our resources are directed towards those in most need we have a selection criteria to determine what problems can be seen in our clinic. The below information is mainly for GPs and other referring clinicians. 

 

Reason for referral must be stated specifically, with supporting information included.

 

Inclusion Criteria

  • Neurological conditions
  • Upper limb conditions due to arthritis, stroke
  • Recent hospitalisation e.g. post-surgery or injury 
  • Current Medicare and/or DVA 

 

Exclusion Criteria

  • National Disability Insurance Scheme participants 
  • Residential Aged Care Facility residents 
  • Worker’s Compensation
  • Compulsory Third-Party Insurance 
  • Transitional Aged Care Package recipients 

 

Out of NSLHD

  • Reason must be stated explicitly  
  • Resident of other LHD that does not provide the clinical service 
  • Continuing care of existing condition already managed by NSLHD 
  • Demonstrated complexity requiring services of NSLHD 
  • Compassionate circumstances, e.g. family, proximity, staff 

 

The following information must be included for referral to be accepted:

  • Referral reason clearly stated
  • Current contact details and/or preferences for contact
  • Communication support needs / Preferred Language to enable NSLHD Translation services,  if required

 

Providing more information will aid the triage process and make it more likely that your patient will be seen in the timeliest manner possible. Outpatient services are delivered at either Brookvale or Mona Vale Community Health Centres, based on availability. Whilst every attempt is made to place clients at their nearest location, scheduling is based on the availability of the OTs. 

 

Please submit referrals using the eReferral system through HealthLink, which can be accessed through most clinical software systems.

 

Telehealth services are available, please indicate on the referral if you feel this would be suitable for your patient’s appointment.
 

 Other information

If you require home-based rehabilitation services, these can be offered to clients over 65 years through the Commonwealth Home Support Program (CHSP) funding for Wellness and Reablement. Clients may include people with disabilities and frail aged needs that can be addressed in up to 6 sessions delivered in the clients home. To access Home Based Rehab you must contact My Aged Care and ask for an Occupational Therapy referral code for Home Based Rehab at either Mona Vale Occupational Therapy or Brookvale Occupational Therapy.
