The Social Work service provides information on, and referrals to a range of Aged Care services including:
- My Aged Care
- Aged Care Assessment Teams
- Support accessing Residential Aged Care
- Community Transport
- Meal Delivery Services
- Carer support services
- Accommodation information
- Financial Support options
The community Social Work team supports People 65 and over or Aboriginal and Torres Strait Islanders who are 50 and over.
We accept community referrals and referrals through the Commonwealth Home Support Program (CHSP).
We have a team of Social Workers who provide inpatient Rehabilitation support to those admitted for rehabilitation. This role encompasses emotional support to patients and their family/carers as well as supporting discharge planning.
