 What we do

  • Treatments include prescription of prosthetics, troubleshooting stump problems, supporting National Disability Insurance Scheme (NDIS) applications, education about prosthetic use and support referral to other services such as physiotherapy or social work. 

 

  • We provide treatment and ongoing care of people who have had leg amputations that offers comprehensive programs to address the physical, emotional, and practical needs of individuals who have undergone limb amputation, with the goal of maximizing their independence, functionality, and overall well-being. 

 What to bring

 When to visit us

Your GP or Specialist is always your first step in seeking treatment for any condition. You will be referred to our service if your treating doctor feels that your treatment requires the input of an Amputee Specialist. Once we receive the referral it will be reviewed, and you will be notified of the outcome and next steps. Please allow at least 4 weeks for this process to be undertaken. Appointments are either face to face or via telehealth - your specialist will let you know what the best option is for you.    

 

Please note that phone calls from the hospital will appear as a blocked number.  Please answer our call if you can and/or turn on voicemail to allow us to leave you a message.  

 

If you have an appointment scheduled, you will receive a reminder text message.  

  

Please allow a full morning or afternoon for your appointment. Some patients require a longer review and delays can occur.  

 How to visit us

Turn onto Palmerston Road from Edgeworth David Avenue. The Emergency entrance is just after the Burdett Street Intersection on the right. The Multi-storey carpark is further along after the Northcott Road junction.

Transdev bus 575 travels to Hornsby Ku-ring-gai Hospital from Hornsby and Turramurra railway stations. This picks up and sets down at the front of the Hospital on Palmerston Road. There are no buses to the Hospital from Waitara Station.

The closest railway stations are Hornsby and Waitara. Hornsby is accessible via both the main Northern Line and the North Shore Line. Waitara is on the North Shore Line. Transdev bus service #575 travels to Hornsby Ku-ring-gai Hospital from these stations.

From Hornsby Station – The distance to the Hospital is approximately 1.8km. If you wish to walk please note there are a number of hills. Exit the station on the George Street side via the pedestrian overpass.


From Waitara Station – The distance to the Hospital is approximately 1.5km. Exit the station on the Alexandria Avenue side. This walk is reasonably flat.

Hornsby Hospital has a multi-storey car park that offers 500 spaces and is located at Gate 2 (Palmerston Road). This carpark provides access for all areas across the campus.

Parking fees apply across the whole hospital in accordance with the NSW Health's hospital car parking fees policy and can be accessed here. Concessional rates apply to those who are eligible.

 

Disabled parking spaces are available in the multi-storey car park and also on Lowe and Palmerston Roads, though street parking availability is very limited. Your valid disabled parking permit must be displayed in your car at all times. Please do not park in restricted parking areas; you could be blocking access to emergency services.

 Referral information

To access the service you will need a referral letter from your doctor, specialist or be a current in-patient in a rehabilitation ward.

 

To ensure that our resources are directed towards those in most need we have a selection criteria to determine what problems can be seen in our clinic. The below information is mainly for GPs and other referring clinicians. 

 

Inclusion Criteria: 

  • Lower limb amputation (upper limb amputations are sent to Westmead or Prince of Wales Hospital 
  • GP referral is required  
  • Specialist Referral 
  • Any location (no local government area restrictions) 
  • Current in-patients in the rehabilitation ward 

 

Exclusion Criteria 

  • No Medicare entitlement 
  • Patients without GP referrals  

 

Out of NSLHD   

  • Resident of other LHD that does not provide the clinical service 
  • Continuing care of existing condition already managed by NSLHD 
  • Compassionate circumstances, e.g. family, proximity, staff 

 

Please submit referrals using the e-Referral system through HealthLink, which can be accessed through most clinical software systems.

 

If unsure which Specialist you would like to refer to, address letters to Dr Cesar Uy or alternatively, address letters to one of the doctors listed in the ‘Other information’ section” 

 

The following information must be included for referral to be accepted:  

  • Name of clinic e.g. Amputee Clinic 
  • Date and duration of referral (GP referrals accepted for either a 12-month period or indefinite)  
  • Referrer's name, address, provider number and signature  
  • Patient's details - name, date of birth, address, phone numbers, Medicare number/Department of Veteran Affairs number  
  • Provisional diagnosis and reason for referral  
  • Finding/treatment to date  
  • Significant medical history  
  • List of medications  
  • Relevant social information  
  • Need for interpreter and preferred language  
  • If applicable, the most recent letter from any other physician, e.g. Cardiologist 

 

Referrals with insufficient information will be returned to the GP/Specialist until further information is provided to the clinic.  Providing more information will inform the triage process and ensure patients are seen in a timely manner. 

   

Referrals will be triaged, and you will be notified of the outcome as soon as possible. This process may take between 1-4 weeks. The GP will be contacted if more information is needed.
