 What we do

The Hornsby diabetes education service provides individual assessment and diabetes self-management education to people with diabetes their family and friends.

 

It is the aim of the Hornsby Diabetes Education Service (DES) to: 

  • Enable the person with diabetes and significant others to minimise the effect of diabetes on daily living, lifestyle, occupation and recreation.  
  • Enable the person with diabetes to develop practical diabetes self-management skills. 

 

Services include:

  • Diabetes self-management education
  • Training in the use of blood glucose monitoring systems
  • Insulin initiation, stabilisation and training in the use of insulin delivery systems
  • Monthly diabetes group education programs
  • Participation in community awareness programs
  • Brooklyn interdisciplinary diabetes clinic

 

The Hornsby diabetes education service provides services to outpatients and inpatients.

 

We care for and treat people with:

  • Gestational Diabetes Mellitus (GDM)
  • Type 1 diabetes
  • Type 2 diabetes
  • Pre-diabetes

 What to bring

  • Relevant Images/scans
  • Blood test results
  • Your Medicare card
  • Your referral letter from your GP/Specialist

 When to visit us

The Hornsby Diabetes Education Service is primarily an adult service. Education and services may be provided to paediatric and adolescent patients on a limited basis. 

 

You may self-refer, however referrals from a health service provider are preferred. 

 

You must have been diagnosed with: 

  • Gestational Diabetes Mellitus (GDM) 
  • Type 1 diabetes 
  • Type 2 diabetes 
  • Type 3c diabetes 
  • Pre-diabetes 

 How to visit us

Turn onto Palmerston Road from Edgeworth David Avenue. The Emergency entrance is just after the Burdett Street Intersection on the right. The Multi-storey carpark is further along after the Northcott Road junction.

Transdev bus 575 travels to Hornsby Ku-ring-gai Hospital from Hornsby and Turramurra railway stations. This picks up and sets down at the front of the Hospital on Palmerston Road. There are no buses to the Hospital from Waitara Station.

The closest railway stations are Hornsby and Waitara. Hornsby is accessible via both the main Northern Line and the North Shore Line. Waitara is on the North Shore Line. Transdev bus service #575 travels to Hornsby Ku-ring-gai Hospital from these stations.

From Hornsby Station – The distance to the Hospital is approximately 1.8km. If you wish to walk please note there are a number of hills. Exit the station on the George Street side via the pedestrian overpass.


From Waitara Station – The distance to the Hospital is approximately 1.5km. Exit the station on the Alexandria Avenue side. This walk is reasonably flat.

Hornsby Hospital has a multi-storey car park that offers 500 spaces and is located at Gate 2 (Palmerston Road). This carpark provides access for all areas across the campus.

The Diabetes Education Service is located on the Ground floor of Building 14 in Hornsby Ku-ring-gai Hospital.

 Referral information

You may self-refer, however referrals from a health service provider are preferred.

 

To ensure that our resources are directed towards those in most need we have a selection criteria to determine what problems can be seen in our clinic. The below information is mainly for GPs and other referring clinicians. 

 

Inclusion Criteria 

  • GP, Specialist or self-referral  
  • Lives within the Northern Sydney Local Health District (NSLHD) 
  • Gestational Diabetes Mellitus (GDM), type 1 Diabetes, type 2 Diabetes, type 3c diabetes and Pre-Diabetes 

 

Exclusion Criteria

  • Out of NSLHD Local Health District 
  • Does not meet the eligibility criteria 

 

Out of NSLHD   

  • Resident of other LHD that does not provide the clinical service 
  • Continuing care of existing condition already managed by NSLHD 
  • Compassionate circumstances, e.g. family, proximity, staff 

 

Please submit referrals using the e-Referral system through HealthLink, which can be accessed through most clinical software systems 

 

The following information must be included for a referral to be accepted:  

  • Name of clinic 
  • Date and duration of referral (GP referrals accepted for either a 12-month period or indefinite)  
  • Referrer's name, address, provider number and signature  
  • Patient's details - name, date of birth, address, phone numbers, Medicare number/Department of Veteran Affairs number  
  • Provisional diagnosis and reason for referral  
  • Finding/treatment to date  
  • Significant medical history  
  • List of medications  
  • Relevant social information  
  • Need for interpreter and preferred language  
  • If applicable, the most recent letter from any other physician, e.g. Cardiologist  

  

Referrals with insufficient information will be returned to the GP/Specialist until further information is provided to the clinic. Providing more information will inform the triage process and ensure patients are seen in a timely manner. 

  

Referrals will be triaged, and your patient will be notified of the outcome as soon as possible. This process may take between 1-4 weeks. You will be contacted if more information is need.
