 What we do

This clinic provides assessment and therapy for:

  • Neurological conditions - stroke, Parkinsons disease, multiple sclerosis, motor neurone disease
  • Arthritic conditions - rheumatoid arthritis, osteoarthritis
  • Upper limb therapy - splinting, reach, grasp and release training, spasticity management, fine-motor therapy, one-handed use, upper limb group
  • Cognition - memory, thinking, planning
  • Functional mobility and transfer training
  • Equipment and aid prescription, compensatory therapy - such as one-handed dressing techniques
  • Self-care training
  • Writing and computer/ technology use
  • Domestic task training
  • Falls prevention and education
  • Back care education

 What to bring

  • Relevant images or scans 
  • List of current medications and drug allergies
  • Your referral letter from your GP/Specialist
  • Your Medicare/DVA Card
  • Wear comfortable clothing and safe footwear.

 When to visit us

Your GP or non-GP specialist is always your first step in seeking treatment for any condition. You will be referred to our service if your treating doctor feels that your treatment requires the input of an Occupational Therapist. Once we receive the referral it will be reviewed by a therapist and you will be notified of the outcome and next steps. Please allow at least 4 weeks for this process to be undertaken. We only see patients face to face.

 How to visit us

Turn onto Palmerston Road from Edgeworth David Avenue. The Emergency entrance is just after the Burdett Street Intersection on the right. The Multi-storey carpark is further along after the Northcott Road junction.

Transdev bus 575 travels to Hornsby Ku-ring-gai Hospital from Hornsby and Turramurra railway stations. This picks up and sets down at the front of the Hospital on Palmerston Road. There are no buses to the Hospital from Waitara Station.

The closest railway stations are Hornsby and Waitara. Hornsby is accessible via both the main Northern Line and the North Shore Line. Waitara is on the North Shore Line. Transdev bus service #575 travels to Hornsby Ku-ring-gai Hospital from these stations.

From Hornsby Station – The distance to the Hospital is approximately 1.8km. If you wish to walk please note there are a number of hills. Exit the station on the George Street side via the pedestrian overpass.
From Waitara Station – The distance to the Hospital is approximately 1.5km. Exit the station on the Alexandria Avenue side. This walk is reasonably flat.

Hornsby Hospital has a multi-storey car park that offers 500 spaces and is located at Gate 2 (Palmerston Road). This carpark provides access for all areas across the campus.

The Occupational Therapy Clinic is located in the Outpatient Department on Level 1. This is entry level as you enter the hospital main entrance.

 

Walk directly forward and you will reach the Outpatient Departme

 Referral information

Access to the service is by referral from GP, rehab specialist, health professional or self.

To ensure that our resources are directed towards those in most need we have a selection criteria to determine what problems can be seen in our clinic. 

 

Inclusion Criteria

  • You must reside in the Hornsby or Ku-ring-gai  local government areas
  • You must be aged 17 years and over
  • You must meet an identified need for occupational therapy.

 

Out of NSLHD

  • Reason must be stated explicitly  
  • Resident of other LHD that does not provide the clinical service 
  • Continuing care of existing condition already managed by NSLHD 
  • Demonstrated complexity requiring services of NSLHD 
  • Compassionate circumstances, e.g. family, proximity, staff 
     

The following information must be included for referral to be accepted

  • Patient name and contact details
  • Diagnosis and medical history 


Providing more information will aid the triage process and make it more likely that your patient will be seen in the most timely manner possible. 

Please submit referrals using the eReferral system through HealthLink, which can be accessed through most clinical software systems.

 

Referrals will be triaged and your patient will be notified of the outcome as soon as possible. This process may take between 1-4 weeks. You will be contacted if more information is needed.  

 

Telehealth services are not available for our patients.
