 What we do

We provide a physiotherapy service in the hospital as well as in the home.

 

We provide individualised physiotherapy services including assessment, education, training, exercise and equipment to optimise the physical ability and maximise the independence of people living in the community.

 

We provide treatment for people who may have:

  • Neurological conditions such as stroke or Parkinson's disease resulting in weakness, poor coordination, impaired balance, decreased movement, or difficulty walking

 What to bring

  • Relevant images/scans/blood test results 
  • Relevant test results (including)
  • List of current medications and drug allergies
  • Your referral letter from your GP/Specialist
  • Your Medicare Card

 

Please wear comfortable clothing and shoes appropriate for walking. Bring your mobility aid (eg walking stick or frame ) if you use one.

 When to visit us

Your GP or non-GP specialist is always your first step in seeking treatment for any condition. You will be referred to our service if your treating doctor feels that your treatment requires the input of a Physiotherapist. Once we receive the referral it will be reviewed by a Physiotherapist and you will be notified of the outcome and next steps. 

 

Please note that phone calls from the hospital will appear as a blocked number.  Please answer our call if you can and/or turn on voicemail to allow us to leave you a message.  


 
If you have a scheduled appointment, you will receive a reminder text message.   

 How to visit us

Turn onto Palmerston Road from Edgeworth David Avenue. The Emergency entrance is just after the Burdett Street Intersection on the right. The Multi-storey carpark is further along after the Northcott Road junction.

Transdev bus 575 travels to Hornsby Ku-ring-gai Hospital from Hornsby and Turramurra railway stations. This picks up and sets down at the front of the Hospital on Palmerston Road. There are no buses to the Hospital from Waitara Station.

The closest railway stations are Hornsby and Waitara. Hornsby is accessible via both the main Northern Line and the North Shore Line. Waitara is on the North Shore Line. Transdev bus service #575 travels to Hornsby Ku-ring-gai Hospital from these stations.

From Hornsby Station – The distance to the Hospital is approximately 1.8km. If you wish to walk please note there are a number of hills. Exit the station on the George Street side via the pedestrian overpass.


From Waitara Station – The distance to the Hospital is approximately 1.5km. Exit the station on the Alexandria Avenue side. This walk is reasonably flat.

Hornsby Hospital has a multi-storey car park that offers 500 spaces and is located at Gate 2 (Palmerston Road). This carpark provides access for all areas across the campus.

Parking fees apply across the whole hospital in accordance with the NSW Health's hospital car parking fees policy and can be accessed here. Concessional rates apply to those who are eligible.

 

Disabled parking spaces are available in the multi-storey car park and also on Lowe and Palmerston Roads, though street parking availability is very limited. Your valid disabled parking permit must be displayed in your car at all times. Please do not park in restricted parking areas; you could be blocking access to emergency services.

 Referral information

A referral from a GP or other clinician is required to access our services.

 

To ensure that our resources are directed towards those in most need we have a selection criteria to determine what problems can be seen in our clinic. The below information is mainly for GPs and other referring clinicians. 
 

Inclusion Criteria 

  • Neurological conditions such as stroke or Parkinson's disease resulting in weakness, poor coordination, impaired balance, decreased movement, or difficulty walking.
  • Lives in Hornsby or Ku-Ring-Gai local government catchment area
  • Medicare card required or will be charged prior to treatment
  • Condition amenable to physiotherapy intervention

 

Exclusion Criteria

  • Anyone under 16 years old 
  • Anyone who can access other private service options E.G NDIS funded, DVA, Level 3/4 Care Package
  • Already seeing a Physiotherapist for the same condition 

 

Out of NSLHD

  • Reason must be stated explicitly  
  • Resident of other LHD that does not provide the clinical service 
  • Continuing care of existing condition already managed by NSLHD 
  • Demonstrated complexity requiring services of NSLHD 
  • Compassionate circumstances, e.g. family, proximity, staff 

 

Providing more information will aid the triage process and make it more likely that your patient will be seen in the most timely manner possible. 

 

Provide patient details: Name, date of birth, address, contact details including email address and phone number.

 

Please submit referrals using the eReferral system through HealthLink, which can be accessed through most clinical software systems.

 

Referrals will be triaged and your patient will be notified of the outcome
