All patients require a GP referral, clearly outlining the reason for referral, listing relevant medical history, previous investigations and current medication list to ensure communication of patient management in the community.
Referral must be received on the day of presentation for consideration and to facilitate timely access to the clinic if eligible.
Indications for urgent referral to local Emergency Department
- Refer patients with acute neurological symptoms thought to be secondary to TIA or stroke directly to the emergency department.
Inclusion Criteria
- Stroke or TIA in the past weeks
- Preference is given to patients who have been cared for in the Royal North Shore Acute Stroke Unit and hospitals in NSLHD. Referrals received directly from GP's or other LHD's will be considered and triaged accordingly.
- TIA patients referred from Ryde Hospital ED are accepted.
Exclusion Criteria
- Patients in Hornsby and Northern Beaches catchment should be referred to the relevant hospital.
Providing more information will aid the triage process and make it more likely that your patient will be seen in the most timely manner possible.
Please submit referrals using the eReferral system through HealthLink, which can be accessed through most clinical software systems.
Advise the patient referral comprises a comprehensive five-to-six-hour visit as an alternative to a three-to-four day inpatient admission. All eligible patients will receive a clinical assessment, stroke protocol MRI of the brain, doppler ultrasound, trans thoracic echocardiogram, holter ECG, and fasting bloods.
