Referrals 

We accept eReferrals from GPs via HealthLink. For more information go to: Electronic Referrals for Outpatient Clinics​ - Northern Sydney Local Health District (nsw.gov.au) 

Opening hours

We have a variety of clinics that run Monday- Friday from 8:30am to 4:00pm for eligible patients.

You will be booked into the most appropriate clinic for your condition.

 What we do

We care for and provide treatment to people who reside within the Lower North Shore Local Government area (Lane Cove, Mosman, North Sydney, Willoughby) for various conditions such as:

 

  • Malnutrition
  • Cardiovascular disease
  • Food Allergy
  • Cancer
  • Gastrointestinal diseases (e.g. inflammatory bowel disease)
  • Paediatric conditions

 What to bring

  • Relevant recent blood tests
  • List of current medications and drug allergies 
  • Your referral letter from your GP/Specialist
  • Your Medicare/DVA Card 

 When to visit us

Your GP or specialist is always your first step in seeking treatment for any condition. You will be referred to our service if your treating doctor feels that your treatment requires the input of a Dietitian and you are eligible. Once we receive the referral it will be reviewed by our service. We ask you to please call the service after receiving your referral to book your appointment. We see patients either face to face or via telehealth, we will let you know what the best option is for you.

Please note that phone calls from the hospital will appear as a blocked number.  Please answer our call if you can and/or turn on voicemail to allow us to leave you a message.  

 

 How to visit us

Exits off the Pacific Highway and the M1 will take you to the hospital on Reserve Road in St Leonards

Bus Route 114 stops directly at the hospital

The hospital is close to St Leonards train station

The hospital is a 5 minute walk from St Leonards Station

Street and multi storey car parking are available on the Royal North Shore Hospital campus. Fees apply, so please check signage. Concession parking rates are available for patients and carers. Click here for more details.

The Nutrition Clinic is located at Check in D of the Ambulatory Care Centre.

 

When you enter the hospital walk past the yellow lifts towards the rear of the building. The Centre is located on the right hand side of the main corridor between the orange and green lifts. Check in D can be found by turning left after you pass the enquiries desk. 

 Referral information

Eligibility:


•    Resides in Lower North Shore local government area (Mosman, Willoughby, North Sydney, Lane Cove)
•    Aged > 18 years
•    Weight management referrals are only accepted if there are multiple comorbidities.
•    Consents to referral

Out of Area Referrals- are they accepted?

 

•    Referrals from other specialists, for specialist opinion – Yes referral accepted at RNSH
•    Resident of other Local Health District that DOES NOT provide the clinical service e.g. rural, outer metro- Yes, but service problem needs to documented on referral.
•    Continuing care of existing condition, we already manage- Yes, provided existing or related condition documented on referral.
•    Demonstrated complexity requiring services of Royal North Shore Hospital Ambulatory Care Centre- Yes but must be explicitly documented on referral.
•    Compassionate circumstance (e.g family proximity, staff)- yes but must be explicitly documented on referral.
•    Resident of other Local Health District that offers the Service- No- refer to your Local Health District

 

Referrals need to be made from a GP or specialist doctor. The following information must be included for referral to be accepted: 


•    Patient’s full name
•    Date of birth
•    Contact details (in case the appointment needs to be changed)
•    Address
•    Reason for referral and nutrition concern/condition
•    Medical History
•    Medication History
•    Preferred language if an interpreter is required.
•    Please also attach any relevant results to the referral.

 
As RNSH provides a range of speciality clinics, please address the referral to “General Dietetics” and the team will assign the appropriate clinic.

It is expected that patients or their carer contact the department on (02) 9463 1666 to make an appointment to see the Dietitian after the referral has been emailed (or faxed) to the department.

 

Providing more information will aid the triage process and make it more likely that your patient will be seen in the most timely manner possible 

 

Please submit referrals using the eReferral system through HealthLink, which can be accessed through most clinical software systems
