 Referral information

Medical referral

Referrals are required to contain patient name, date of birth and contact details (in case the appointment needs to be changed), medical history, medication history, reason for referral and nutritional concern.

It is expected that patients or their carer contact the department to make an appointment to see a dietitian after the referral has been emailed (or fax if email is not available) to the department. The referral is required prior to booking an appointment so that the person can be booked into the most suitable clinic for their nutritional need.

We are sorry, however due to the volume of referrals received, the department is unable to contact patients to make appointments.

 Other information

Is there a cost to patients to attend the clinic?

No, excepting for people who are not covered by Medicare when there will be a charge for this service
