BRACE (Beaches Rapid Access Care of the Elderly) - Mona Vale Hospital

The Brace team is a multidisciplinary geriatric outreach team providing care for frail elderly people living in the community or in a Residential Aged Care Facility

We work together with GPs to provide initial assessment and treatment for:
•    infections: urinary, pneumonia, gastroenteritis, cellulitis
•    falls
•    simple fractures
•    acute episodes of heart failure
•    acute episodes of a chronic lung condition eg emphysema
•    recent confusion (delirium) due to illness
•    behavioural support for dementia
•    administration of IV medications
•    acute on chronic pain
•    medication reviews where acutely indicated
•    support for catheter management
•    acute/deteriorating wounds
•    short term care coordination
•    short term end of life care

To be eligible for this service consumers need to be:

• Frail elderly (70+) or younger with geriatric syndromes
• Living in the Northern Beaches LGA
• Experiencing an acute medical/functional/cognitive decline
• At risk of imminently presenting to hospital
