Care tailored for you
At regular team meetings we discuss the management of your care, your progress, future goals and the plans for your discharge.
A home visit assessment may be carried out by our occupational therapists who can facilitate home modifications and equipment if required.
When returning home, you and your GP are provided with a discharge summary and a medication list. A pharmacist will review and discuss your medication requirements.
Your family and carers may visit at any time and are welcome to be involved in your therapy sessions.
