In Northern Sydney Local Health District (NSLHD), residents have timely access to high quality supportive and palliative care services.

NSLHD provides supportive and palliative care services in acute hospitals, sub-acute palliative care units, in people's homes and residential aged care facilities. People may receive care in some or all of these settings, depending on their illness trajectory and care needs.


Our service approach

  • Admitted palliative care is delivered by specialist palliative care doctors, nurses and allied health professionals in the sub-acute palliative care units and through hospital-based palliative care consult services.
  • Non-admitted palliative care is delivered through community palliative care services and specialist palliative care outpatient clinics, provided by a mix of multidisciplinary and single discipline clinics.
  • Various supportive and palliative care services are delivered in collaboration with HammondCare under a local service-level agreement based on agreed partnership principles.


HammondCare partnership​


NSLHD and HammondCare have established a partnership to co-deliver a comprehensive range of supportive and palliative care services for residents of NSLHD.​
