Costs

Requests for information from insurers or other parties must be accompanied with a valid consent and monies paid before any information is released.

 

Fees for copies of notes are $33.00 for the first 80 pages (inc. GST). Notes in excess of 80 pages are charged at $0.41 per page.

 

Cost recovery for the provision of other material e.g. reproduction of X-rays, audio-visual tapes, copies of photographs and operation footage contained on DVD's.

 

Fees for medical reports range from $329 - $846 dependent on category (see NSW Health Charging Policy - Health Records and Medical/Clinical Reports)

Subpoenas

​Subpoenas for patient records held by a Northern Sydney Local Health District hospital should be addressed to the Health Information Services Manager of the hospital that holds the records.

Note that subpoenas to appear to court should be served directly on the person.

Conduct Monies

The party issuing the subpoena shall be charged a per patient per hospital fee for requests addressed to NSLHD.

Where:

  • At least 5 working days' notice is given for the production of the record to Court ($75)
  • Less than 5 working days' notice is given for the production of the record to Court ($113)

An additional photocopying fee of $0.41 per page is also applicable.

Chapter 16A central contact point

​Northern Sydney LHD has a centralised service point that receives and responds to Chapter 16A requests for the Northern Sydney Local Health District.

 

Under Chapter 16A of the Children and Young Persons (Care and Protection) Act 1998, an organisation that is a prescribed body can be requested to provide and share any information it holds relating to the safety, welfare or wellbeing of a particular child or young person.

 

Information exchanged between prescribed bodies under Chapter 16A is preferably done so verbally, between a worker in a prescribed agency and a Health worker.

 

Prescribed bodies include services that work with children and young people or their families and include but are not limited to State Government agencies, Non-Government Organisations (NGOs), Aboriginal Medical Services, education providers and child care centres and Medical Practitioners.
